Food Safety & Drinking Water Forms (Applications/Exemptions)

Food License Application (pdf)

This is the license application that must be submitted to SDHD for a food license.  This application is used for all types of food establishments.

Fraternal/Benevolent/Charitable Organization
Exemption form

This is a dual purpose exemption form.  It is used for establishments that want to sell low-risk food or Fraternal, Benevolent, or Non-Profit organizations that wish to sell no more than one meal per week.

Drinking Water Well Construction Design Checklist (pdf)

This form is supplied by DEQ (Department of Environmental Quality) and is one of two required forms that must be submitted to DEQ to have a private well approved as a public drinking water system. 

Public Drinking Water Well Site Evaluation (pdf)

This form is supplied by DEQ (Department of Environmental Quality) and is one of two required forms that must be submitted to DEQ to have a private well approved as a public drinking water system.
